If you have registered for an online service account, you can reschedule your annual appointment through the online service.
Please note:

  • To activate your account you need an Activation Code and a Booking System Registration Number
    • The activation code can be obtained from your designated Student Health Service Centre when attended the annual appointment. (illustration A)
    • *The Booking System Registration Number (BSRN) may refer to the label on the front page of the Child Health Record. (illustration B)

    *BSRN provided by the Student Health Service Centre since November 2024. (illustration B)

  • As the quota of appointments are limited, parents/students can only reschedule their appointment up to two times within this school year.
