Rescheduling Request

If you would like to apply for rescheduling appointment, please click the “Rescheduling Request” button below and fill in the student's name, designated centre and contact phone number. We will contact you in 7 working days for follow-up.

If we are unsuccessful in contacting you for 3 times at different sessions, your application will be closed. If necessary, please re-submit the application for rescheduling.

Two rescheduling requests are only allowed for students / parents who need to reschedule the annual health appointment.

If you have registered an online service account, you can change the annual appointment through the online service.
