Enrolment document for the school year of 2026/27

The Student Health Service of the Department of Health aims to safeguard both the physical and psychological health of students through health promotion and disease prevention services. Our scope of service includes health assessment, (including physical examination, growth and puberty assessment when necessary), individual health counselling and health education. For details of the health programmes categorised by grade, you may refer to the “Health Programmes at Student Health Service Centre”.

Enrolment Method

1. Local students can enrol through their schools at the beginning of each school year in September.
2. If students cannot enrol through their schools, OR Hong Kong residents who are studying Primary 1 to Primary 6 in overseas / Chinese Mainland, please refer to the “Notice to Parents/Guardians (Primary)” and submit the application through the following methods:
   
   Documents required:
   
   • 1. Completed “Application & Consent Form for Student Health Service 2026/27”
     (For enrolment of Student Health Service only.
     If you wish to join the School Dental Care Service, please contact your child's school or your school designated School Dental Clinic to obtain the enrolment form.)
   • 2. Copy of the student’s identity document, and
   • 3. Copy of school certification (The document must indicate the 2026/27 academic year, school name, student's full name, grade and class, and bear the official school seal).
   
   Submission methods:

   • 2. By fax: (Designated Student Health Service Centre)
   • 3. By email: <dh_stdhs_enrolment_exercise@dh.gov.hk>
   • 4. “iAM Smart+”users may submit through GovHK
     (Online applications will be accepted from 1 October 2026 to 25 August 2027)

The service is provided free to those students who are “eligible persons”. The fees payable for non-eligible persons is HK$680 (please refer to the “Guidelines to Verification of Eligibility” for details).

Enrolled students will be given an annual appointment at a (Designated Student Health Service Centre). The service period starts from November 2026 to October 2027.
