What is the Student Reference Number (STRN) for the Internet service?

  • In general, the STRN for Student Registration is the registration number of your child's Hong Kong Birth Certificate or the Hong Kong Identity Card number (including the number/alphabet in the bracket but without the bracket) or Re-entry Permit number.

What is the Activation Code for the Internet service?

  • To obtain an Activation Code, you have to sign the Consent and Declaration of the "Application form for Joining Internet Services of the Student Health Service (Parent)" which is printed on the back page of the given Appointment Letter.
    Then submit the consent form to the doctor or nurse when your child attends the Student Health Service Centre for annual health assessment. After receiving your consent, an Activation Code will be generated to you / your child on the same day which is printed onto the "Personal Health Assessment Findings and Recommendations".
    Access our website https://www.studenthealth.gov.hk/wsmash/registerStep1.jsp, key in the Student Reference Number (STRN), Date of Birth and the Activation Code to register as a user.
    Primary 1 or students who had never joined our Service will not have the Activation Code.

How to re-schedule the appointment date or time?

  • Upon receipt of the appointment letter, you could call the staff of the designated Centre to make arrangement. Please refer to Centre Information for details.
    You can also re-schedule your appointment in Student Login via the Internet service if you have already registered. (Remark: Same day appointment cannot be re-scheduled via the Internet service).

Can the student change the Student Health Service Centre (SHSC) for annual assessment?

  • Yes, upon receipt of the appointment letter, you could call the staff of the designated Centre to make arrangement. Please refer to Centre Information for details.

When will I receive the appointment letter?

  • Student Health Service Centre will inform schools to take the appointment letter after the appointment is arranged. Schools then distribute the appointment letter to students about one month before the appointment.

I have re-scheduled the appointment via phone, but why the Internet service does not show the updated appointment?

  • As the synchronization of data of centres and our website is scheduled after office hours, registered users can access our website to get the latest information after 7:30pm of every working day.

I have pressed “Forget Password” on the Internet service, but why still cannot receive the email for changing the password?

  • The Internet service updates after office hours. Please check the email you registered for the Internet service after 7:30pm of every working day.

What will be assessed in the annual assessment?

  • The Health Programmes of SHSC include physical examination; screening for health problems related to growth, nutrition, blood pressure, vision, hearing, spine, psychological health and behaviour; individual health counselling and health education.

Enquiry about Immunisation/Vaccination of Primary School students.

  • Please call the School Immunisation Teams on 2615 8585 / 2615 8563.
    (As SHSC provides mop-up immunisation for Secondary Students ONLY.)

  • Please call the staff of the designated Centre to enquire or make arrangement. You may refer to Centre Information for details.

Having lost the Immunisation/Vaccination Card/Record, how to re-issue it?

  • Student / Parent can download the forms from [Application for Copy of Childhood Immunisation Record]. The completed application form and the relevant documents can be submitted in person or by post to either one of the Student Health Service Centres, Maternal and Child Health Centres or School Immunisation Teams.

Can overseas students who are born in HK join the Student Health Service?

  • Hong Kong residents who study in Primary 1 to Secondary 6 in overseas/ Mainland China schools can join this service by individual enrolment.
    Please obtain the Application and Consent form from the centre OR our website. Fill in the form and return to the centre you selected by fax, post or in person. You may refer to Centre Information for details.

New immigrants arrive HK in the middle of school year, can they join this service?

  • Yes. Students who study in Primary 1 to Secondary 6 day schools can join the service by individual enrolment.
    Please obtain the Application and Consent form from the centre OR our website. Fill in the form and return to the designated centre by fax, post or in person. You may refer to Centre Information for details.
