What will be assessed in the annual assessment?
- The yearly health assessment of Student Health Service include screening for health problems related to growth, nutrition, blood pressure, vision, hearing, spine, psychological health and behavior; individual health counselling and health education.

When will I receive the appointment letter?
- Student Health Service Centre will inform schools to collect the appointment letter at designated centres about 3 to 4 weeks after the appointment is arranged. Schools then distribute the appointment letter to students before the appointment.

Apart from appointment letter, is there any other means I can receive the appointment date?
- Apart from the appointment letter, student/parent will be notified 4 working days before the appointment date by receiving a SMS reminder sent by Student Health Service, the Registered SMS Sender IDs will be marked with “#DH-CLINICS”.

What are the registration hours and arrangements for Student Health Service?
- The registration hours of Student health Service Centres under the Department of Health are Monday to Friday 8:45 AM to 12:15 PM and 1:30 PM to 5:15 PM (except public holidays). Please arrive at the centre 15 minutes before the appointment date and time for registration. Late comers over 30 minutes may not receive the service on that date. Please be aware that the appointment time does not represent your time of service.

How to re-schedule the appointment date or time?
- Upon receipt of the appointment letter, you could call the staff of the designated Centre to make arrangement if genuinely needed, or visit Student Health Service website, click the “Rescheduling Request” button and fill in the student’s name, designated centre and contact phone number. Centre staff will contact you in 3 working days. Please refer to Centre Information for details.
As the appointments are arranged in advance, centre may not provide the appointment you want, please attend the appointment as much as possible.
You can also re-schedule your appointment in
Student Login via the Internet service if you have already registered. (Remark: Same day appointment cannot be re-scheduled via the Internet service.)

What is the new arrangement of rescheduling annual appointment?
- Two rescheduling requests are only allowed for students / parents who need to reschedule the annual health appointment. You can contact the designated centre to reschedule the date and time slot that you want to change for the first time according to the centre’s situation.
If second time of appointment rescheduling is required, your appointment will be arranged by centre staff.
Any missed appointment as scheduled and need to reschedule, it will be regarded as the appointment has been changed once.

Can the student change the Student Health Service Centre (SHSC) for annual assessment?
- Enrolled students were allocated to the designated Student Health Service Centre in term of their school district and student population for annual assessment. For better service utilisation, starting from 2024/2025, the quota of changing service centre would only be provided for students who change to new school or assign the siblings for attending the same service centre for annual assessment. In view of the overwhelming needs, only limited quotas can be allocated for requests on changing centre. Since there are many students who studying across districts, we would not accommodate the requests of changing centre with reason of students’ residential address.

Can overseas students who are born in HK join the Student Health Service?
- Hong Kong residents who study in Primary 1 to Secondary 6 in overseas/ Mainland China schools can join this service by individual enrolment.
Please obtain the Application and Consent form from the centre OR our website. Fill in the form and return to the centre you selected by fax, post or in person. You may refer to Centre Information for details.
(Remarks: Primary 1 to Secondary 6 students studied in overseas/ Mainland China need to submit the original school attendance certificate and copy of identity documents for application.)

- Students who study in Primary 1 to Secondary 6 day schools can join the service by individual enrolment.

Having lost the Immunisation/Vaccination Card/Record, how to re-issue it?

Parent / Student can download the forms [Application for Copy of Childhood Immunisation Record] from Student Health Service website. The completed application form and the relevant documents can be submitted in person or by post to either one of the Student Health Service Centres, Maternal and Child Health Centres or School Immunisation Teams or email to dh_vac_copy@dh.gov.hk
You may also apply for a copy of immunisation record by online application from GovHK. Upon successful submission, you will be issued a reference number. The chosen pick-up centre will make arrangements and communicate with you accordingly.

What can I do if the adverse weather conditions happen on the appointment date?
- When Tropical Cyclone Warning Signal No. 8 or above is hoisted, or “Extreme Conditions” announced by the Government or Black Rainstorm Warning Signal is / are in force, services at our centres will be suspended. Please pay attention to adverse weather conditions and Government announcements on relevant public services. After resumption of service, please contact centre staff for rescheduling of appointment.

What is the Student Reference Number (STRN) for the Internet service?
- In general, the STRN for Student Registration is the registration number of your child's Hong Kong Birth Certificate or the Hong Kong Identity Card number (including the number/alphabet in the bracket but without the bracket) or Re-entry Permit number.

What is the BRSN for the Internet service?
- Students who had joined and attended our Service will be provided a BRSN. Access our website:
